Please see the below items that will be checked during your laser safety inspection. Reviewing the checklist can help you prepare for your inspection.

Laser Posting, Labeling and Security Measures

  • Entrances properly posted

  • Optical Density (OD) listed on sign

  • Room security adequate

  • Door interlock system

  • Laser indicator outside room

  • Laser indicator outside room functional

  • CDRH Laser class label in place

  • CDRH Laser hazard label in place

  • CDRH Laser aperture label in place

Laser Unit Safety Controls

  • Protective housing in place

  • Interlock on housing

  • In-­‐house built enclosure labeled

  • Beam shutter present

  • Key operation

  • Activation indicator on console

  • Beam power meter available

  • Emergency shutoff (EPO) available

  • Emergency shutoff (EPO) tested in last 6 months

Engineering Safety Controls

  • Laser secured to table

  • Laser optics secured to prevent stray beams

  • Laser beam at eye level

  • Upward directed beams

  • Beam path enclosed

  • Beam barriers in place

  • Appropriate barrier material used

  • Beam stops in place

  • Microscope used for viewing (example -­‐ TIRF)

  • Microscope eye pieces filtered

  • Remote viewing of beam (ex.CCD camera)

  • Beam focused or enlarged

  • Beam intensity reduced through filtration (ND filters)

  • Fiber optics used

  • Beam paths free of clutter

  • Physical evidence of stray beams

  • Beam stops in place

  • Microscope used for viewing (example -­‐ TIRF)

  • Electrical panels unobstructed

  • Optical table/equipment grounded

  • Collateral radiation hazard

  • LGAC production

  • Good housekeeping

  • Eye wash/shower area free of clutter

  • Eye wash/shower checked
