Two New Rules in Effect Beginning Monday, June 22, 2009
  1. Hazardous material shipments from the Central Campus must be made only from authorized sites. FedEx will only pickup hazardous materials from these locations:
  2. 156 Stanley Hall
    B84 Hildebrand Hall
    111 Koshland Hall
    1005 Valley Life Science Building
    Note: Please call for assistance if you are not a certified hazardous materials shipper or you are shipping hazardous materials from Richmond Field Station or another field station.

  3. Only properly prepared packages bearing the Hazardous Materials Shipping Label (shown below), marked with the shipper's name, phone number and hazardous materials training certificate number will be picked up by FedEx. This label indicates that the package was prepared by an appropriately trained person.

Note: Personnel at an authorized pickup location and FedEx will check all packages for this label. Packages without the label will not be shipped. Personnel at the authorized pickup locations will not prepare your shipment for transportation. Swine Influenza

Shippers of specimens collected directly from humans or animals that are suspected or confirmed to be infected with the swine influenza A(H1N1) virus, including specimens from the respiratory tract (swabs) and blood specimens, should be shipped as "BIOLOGICAL SUBSTANCE, CATEGORY B" and assigned to UN 3373.

Swine influenza A(H1N1) virus cultures(i.e. virus isolates) must be shipped as Category A "INFECTIOUS SUBSTANCE; AFFECTING HUMANS" and assigned to UN 2814.

Note: Shipping these substances require shippers to have completed specific training. If the shipment also includes other dangerous goods (such as liquid nitrogen or dry ice), shippers must be trained and certified to ship.

Shippers Declaration of Dangerous Goods - Outdated Versions

Certain hazardous materials/ dangerous goods shipments require the use of a Shippers Declaration of Dangerous Goods, easily identified by its red hash marked border. A number of outdated versions have been found on campus.

Please, check your forms. If you purchased a specification performance package that contained a copy of a Shippers Declaration of Dangerous Goods, if you have an inventory of these forms, or a version that you’ve saved on a digital file, check them now!

The following declaration is what you should find near the bottom section of your form.

I hereby declare that the contents of this consignment are fully and accurately described above by the proper shipping name, and are classified, packaged, marked and labeled/placarded, and are in all respect in proper condition for transport according to applicable international and national governmental regulations. I declare that all of the applicable air transport requirements have been met.


If your form does not include this last declaration "I declare that all of the applicable air transport requirements have been met," then your form is outdated and must be destroyed.
